1.0 Objective
To describe the procedure for medical checkup of the all employee
2.0 Scope
This procedure is applicable for medical checkup of the employee factory premises at abc company.

5.0 Procedure
5.1 Each personnel shall undergo medical examination to ensure good personnel health.
5.2 Three main types of medical examination shall be conducted to ensure personnel health.

These are at the time of joining, regular medical examination and rejoining after a long

medical leave (more than ten consecutive working days).
5.3 All employees shall be subject to medical examination before being employed and thereafter

at least once in a year by medical practitioner(s) on the panel of the Company.
5.4 Medical Examination at the Time of Joining
5.4.1 Medical examination shall be conducted at an authorized hospital from nearby area.
5.4.2 All new employees before joining the Company shall be sent to the authorized Hospital for

pre-employment medical examination duly approved by Incharge, HRD. Duly Certified Medical

Fitness Certificate along with relevant test reports, if any, shall be forwarded to the Company for records.
5.4.3 The medical examinations shall include, but not restricted to, X-Ray and Chest examination,

tuberculosis, blood study, presence of skin disease, presence of communicable disease and

contagious disease, urine examination etc.
5.4.4 On being found medically fit, the new joinee employees shall be allowed to join the services

of the Company and their continuity in the service of the Company shall subject to being declared

medically fit in the subsequent medical examinations. Every employee shall fill-in his or her own

handwriting a “Medical Declaration” form (Annexure-I) before medical examination giving the

personal details and previous medical history. After completion of medical examination, the authenticated

reports on the formats shall be provided to the Company.
5.5 Regular Medical Examination
5.5.1 All employees shall undergo regular medical examination.
5.5.2 For annual medical examination, Incharge, HRD shall prepare a list containing names of employees.

Thereupon, suitable date, time and venue of medical examination shall be fixed with the panel of doctors for medical investigation.
5.5.3 The medical examinations shall include, but not restricted to, X-Ray and chest examination.

Tuberculosis, blood study, presence of skin disease, presence of communicable disease and contagious

disease, urine examination, etc. the absence of disease or illness shall be confirmed.
5.5.4 If any employee is required to undergo further medical examination, then it shall be done

at the Authorized Hospital and all relevant reports shall be forwarded to the Company specifying

result(s) of such tests. However, all such employees who are covered under ESI scheme shall be

referred to ESI dispensary for medical examination/treatment.
5.5.5 Any unacceptable state of fitness shall be mentioned and appropriate action taken and recorded.
5.5.6 All employees shall be instructed to report their illness or abnormal health condition to their

immediate supervisor so that appropriate action can be taken.
5.5.7 When an employee becomes ill, they are not permitted to report for work or continue performing

their duties; instead, they must “leave” the workplace right away and be noted as such in the

Attendance Register. If an employee sustains a significant illness or injury, they will be brought to

the closest hospital or one of the company’s panel doctors, whichever is appropriate..
5.6 Medical Examination after Rejoining after a Medical Leave (More than 10 Consecutive Days)
5.6.1 Employees rejoining after medical leave for more than ten consecutive days shall be

re-examined by the company appointed medical practitioner.
5.6.2 The medical examinations shall include, but not restricted to, X-Ray and chest examination.

Tuberculosis, blood study, presence of skin disease, presence of communicable disease and

contagious disease, urine examination, etc. the absence of disease or illness shall be confirmed.
5.6.3 The HRD personnel / Employee shall get Medical Examination Record (Annexure – II) filled from medical practitioner.
5.6.4 Re-joining of duty shall be subjective to successful medical examination.
